Source of referrals and criteria of selection 

Referrals, more often than not, are made by the Integrated Care Services (ICS) to channel clients from various public and community hospitals to MHCC. However, some of the clients are referred to us directly by community hospitals for relevant step-down services. Caregivers do send to us, direct, their loved ones who are recommended by their doctors.  The criteria by which MHCC accepts and receive its clients are as follows: 

·        Frail, but semi-ambulant with walking aid

·        Mentally stable

·        Not suffering from infectious disease

·        Independent in feeding and toileting (if unaccompanied by maid)

·        Does not require 24-hour close supervision. 

Assessments will be made on the individual client on his/her cognitive, physical, mobility and other relevant status.
